Patchwork Community Outreach (PCO) is a Medi-Cal approved Community-Based Organization (CBO) dedicated to empowering individuals facing barriers to care, complex social needs, and challenges accessing healthcare and community resources. Our mission is to help people rebuild meaningful, self-directed lives through compassionate, evidence-based Community Health Worker (CHW), Enhanced Case Management (ECM), and Community Supports (CS) services.

We provide trauma-informed, culturally responsive, person-centered services that promote dignity, resilience, autonomy, inclusion, and hope. Guided by the nationally recognized Recovery Model, we believe recovery is possible for everyone—regardless of diagnosis, circumstance, or life experience.

Our CHWs serve as trusted bridges between individuals, healthcare providers, managed care plans, and community resources. Through outreach, health education, care coordination, resource navigation, advocacy, and practical support, we help people overcome barriers, address social determinants of health, achieve personally meaningful goals, and improve their overall health and well-being.

Whether someone is navigating a new diagnosis, managing a chronic condition, experiencing housing instability, or simply needs support accessing services, PCO meets people where they are and walks alongside them on their journey toward recovery and wellness.
