Patchwork Community Outreach's mission is to empower individuals facing barriers to care, complex social needs, and challenges accessing healthcare and community resources to rebuild meaningful, self-directed lives. We provide trauma-informed, culturally responsive, evidence-based Community Health Worker (CHW) services that promote dignity, resilience, autonomy, inclusion, and hope.

Guided by the nationally recognized Recovery Model, we affirm that recovery is possible for everyone, regardless of diagnosis or history. Our Community Health Workers serve as trusted bridges between people, healthcare systems, and community resources. They offer outreach, health education, care coordination, navigation support, and practical assistance grounded in recovery principles. Through strength-based, person-centered, and member-driven approaches, they help individuals address barriers, meet social needs, pursue personally meaningful goals, and improve their overall health and well-being.
